• Completed submission proforma identifying key contacts and dates.
• Design Statement in line with the enclosed guidance, and a note of the persons (name and role) involved in the development of the statement - i.e. those stakeholders represented in the development of both the agreed non-negotiables and the benchmarks.1
• Commitment to BREEAM Healthcare
