(a)  Required Outcome

Successfully supporting the Participants to plan Community Services across the Territory.

(b)  Scope

(i)  hubco shall work with the Participants to plan community health care, social care and other Community Services that meet the needs of the population in the Territory.  

(ii)  hubco shall support the Participants in the planning of community services across the Territory to:

(1)  help identify opportunities to improve the efficiency and cost effectiveness of the services to ensure they deliver value for money; and

(2)  help support the Participants' achieve their sustainability aims and targets in the delivery of community services.
