16.2  Reporting of Accidents and Unsafe Events

The Service Provider shall:

(a)  ensure that all accidents to Personnel which ordinarily require reporting in accordance with the Health and Safety at Work etc Act 1974 shall also be reported, as soon as practicable, to the Authority; and

(b)  promptly, upon it becoming so aware, provide the Authority with full details of any significant unsafe event which relates in any way to the Service.
