On Wednesday morning, at Leisure Facility Y, a customer complains about toilet paper on the floor of the toilets in the dry changing rooms. It is still there on Friday morning when a duty officer carries out an inspection.

This is recognised as a cleaning failure and a Performance Deduction is to be made as follows;

Performance Standard Weighting (PW) = 5% for Cleaning - Routine

Zone Weighting (ZW) = 5% relating to the Dry Changing Facilities

Event Category (EC) = C = 100%

Contract Days (OD) = 362

PD1 = (UCn x PW x ZW x EC)       x R

Monthly Deduction is £1,000,000 x 5% x 5% x 100% divided by 724 = £3.45 (operational period charge)

£3.45 x 4 operational periods (2 contract days) = £13.80
