Leisure Facility Y fails to achieve Quest Accreditation until 18 months following the Certification of Service Availability. This is a failure to achieve Quest Accreditation within the stated first 12 months and an Annual Performance Deduction is to be made as follows;

Unitary Charge (UCn) = (say) £1,000,000

Performance Standard Weighting for Quest Accreditation = 5%%

Number of Contract Days where Quest accreditation not held in the previous Contract Year (DSNA) = 181

Contract Days (OD) = 362

PD2 =    (UCn x PW)

Annual Deduction is £1,000,000 x 5% x 181 divided by 362 = £25,000 (annual charge).
