(a)  Amend paragraph (a)(4)(i) to remove "and" after the semicolon;

(b)  Amend paragraph (a)(4)(ii) to add the word "and" after the semicolon;

(c)  Add new paragraph (a)(4)(iii) to read as set forth below.
