(If the individual making the unauthorized commitment is a DISA employee, this section is to be completed by the individual's and supervisor's appropriate organizational Deputy Director (DD) or Principal Director (PD), and forwarded to the appropriate contracting activity, contracting officer or contracting office for review and completion.
